Get your recommendation from anywhere with our teledoc service! This option does not include the processing of your information to the state. If you would like that service please choose the other option and visit us at our location in Edmond. ALL PATIENTS MUST HAVE ON OKLAHOMA ID OR DRIVERS LICENSE. Either one of these will qualify as your proof of residency and your proof of identity. However, if you are unable to obtain one of these, OMMA will accept an Oklahoma Tribal ID or a US Passport. With these, you must provide proof of residency such as a utility bill from the month prior, or a current lease agreement. Payment is collected in full when you book.
